Job Summary
Manage a varied caseload of adults with social care needs, including complex cases. Complete Care Act assessments, reviews, and reassessments while developing person-centred support plans to promote independence and wellbeing. Undertake safeguarding enquiries, manage risk, and complete Mental Capacity Assessments. Support hospital discharges to prevent unnecessary admissions and work collaboratively with health services, housing providers, and voluntary organisations. Provide advice to adults and carers, maintain accurate case records, and contribute to service development. Support and supervise less experienced colleagues where appropriate.
Required Qualifications
- A recognised Social Work qualification
- Current Social Work England (SWE) registration
- Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
- Good knowledge of the Mental Capacity Act and relevant legislation
- Strong multi-agency partnership working skills
- Excellent communication, organisational, and problem-solving abilities
- A compassionate, proactive, and person-centred approach to practice
- A commitment to delivering high-quality outcomes for adults and their families
- Must be able to lift 50 lbs
