Social Worker

Social Worker role providing direct service delivery and care coordination within an IPCT-led primary care team across multiple sites in York Region and South Simcoe. Responsibilities include coordinating care plans for patients with mental health and substance use disorders, collaborating with PCPs/IPCT to tailor individualized care plans, assessing social determinants, facilitating warm transfers to external services, and advocating for clients. Responsibilities also cover addictions support, therapeutic use of self, risk assessments, timely EMR documentation, and participation in supervision, performance reviews, and professional development. The clinic emphasizes population health management, equitable access to care, trauma-informed and Indigenous-focused approaches, and supports training to deliver such care. Requirements include an MSW/B.S.W., active SW registration, community-based mental health experience, EMR proficiency, strong communication, cultural sensitivity, a Canadian driver’s license and access to a vehicle. Working conditions involve office and travel with clients, occasional evenings/weekends, and potential mobile clinic operations across Aurora, Keswick, and travel within York Region/South Simcoe.
