Social Worker

As Social Services Designee at Keyser Healthcare Center, you will develop rapport with direct caregivers to support programs/services for residents’ psychosocial well-being, report grievances to the Director of Social Services, advocate for residents, monitor changes in conditions and communicate with the RN Charge Nurse, provide information to help solve residents’ needs, collaborate with the activity department to involve families, maintain confidentiality, and perform general social services duties. The role emphasizes resident advocacy, interdepartmental collaboration, confidentiality, and improving quality of life for residents within a post-acute care setting.
