Job Summary
Social Worker in inpatient Care Management at Rice Memorial Hospital. Provide patient and family support, collaborate with an interdisciplinary team (nurses, therapists, physicians, dieticians), manage a designated patient caseload, plan care to meet transitional needs, monitor length of stay, and promote community resources. Supervisory support is available for new grads; external supervision up to $175/month may be reimbursed. Schedule is full-time, onsite day shift (7:30 AM–4:00 PM, four days/week). Pay ranges from $30.65 to $45.89 per hour, with starting pay tier by degree; $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates. CentraCare is an EEO/AA employer.
Required Qualifications
- Current Licensed Social Worker (LSW) in Minnesota required.
- Bachelor's or Master's degree in Social Work.
