Job Summary
Provides Service Facilitation for CCS, CCOP, and CLTS; conducts Functional Screen; orients families to programs; builds collaborative teams; completes comprehensive consumer assessments; schedules and facilitates team meetings to develop recovery-oriented service plans; monitors progress; coordinates services with families, providers, and community agencies; completes program paperwork; develops crisis/safety plans; arranges appropriate placements; conducts home/community visits; provides consumer transportation; emphasizes recovery-oriented, person-centered approaches and coordination with multiple child/family services in Portage County.
Required Qualifications
- Bachelor degree in social work or any other human services field required
- Two years’ experience working primarily with children and families, including programs dealing with mental health or AODA issues, within the last ten years
