Social Worker

Provide social services for residents and act as a liaison between medical staff, families, and outside agencies. Interview residents and families for social histories while ensuring social and personal aspects of care are properly managed. Collect and update information on community health and welfare resources. Assist with admissions and discharges and conduct care plan and discharge meetings. Requires a Bachelor's degree in social work with preferred long-term care experience and Point Click Care familiarity.
