Social Worker Case Manager

Job Summary

Required Qualifications

• •Master's Degree in Social Work
• •Three (3) years' recent, related experience in case management working with geriatric clients and adults with chronic and acute illness or an equivalent combination of education, training and experience
• •Current full registration with the British Columbia College of Social Workers
• •Valid BC Driver's License
• •Local area travel requires the use of personal vehicle
• •Demonstrated knowledge and demonstrated ability to apply intensive case management principles
• •Demonstrated knowledge and demonstrated ability to apply chronic disease management and health promotion principles
• •Demonstrated knowledge of case management practice within a client and family centred care model for clients with complex health care needs
• •Demonstrated knowledge of Indigenous Cultural Safety Education
• •Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention
• •Demonstrated ability to conduct social and psychosocial assessments
• •Demonstrated knowledge of systems thinking as it relates to the social work process
• •Demonstrated ability to utilize mediation, conflict resolution and counselling skills
• •Demonstrated knowledge of evidence based practice related to care of clients needing supportive case management
• •Demonstrated knowledge of other health disciplines and their role in client care
• •Demonstrated knowledge of other health and community resources related to the care of clients and care givers
• •Demonstrated knowledge of adult education principles, methods and tools
• •Broad knowledge of research process and methodology
• •Demonstrates ability to problem solve using sound judgment in applying critical thinking skills within safe limits of client care
• •Demonstrated ability to provide leadership and work direction
• •Demonstrated ability to teach, coach and facilitate clients and families/care givers/care givers individually or in group settings
• •Demonstrated ability to develop and implement project action plans and accomplish objectives
• •Demonstrated ability to work independently
• •Demonstrated ability to work collaboratively as a member of a interdisciplinary team
• •Demonstrated ability to effectively communicate orally and in writing
• •Demonstrated ability to communicate with, and deal effectively with co-workers, physicians, other health care staff, clients and their families/care givers, and staff of outside agencies
• •Demonstrated ability to effectively deal with conflict resolutions
• •Demonstrated ability to adjust to new or unexpected events
• •Demonstrated ability to plan, organize and prioritize work in collaboration with others
• •Demonstrated ability in conducting psychosocial/functional environmental assessments
• •Demonstrated skill and ability in the use of equipment, supplies and materials utilized in the provision of community based care service
• •Basic computer literacy to operate a computerized client care information system
• •Physical ability to carry out the duties of the position
• •All health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections
