The Social Worker Case Manager is responsible for developing and coordinating inpatient discharge plans for patients at risk for adverse outcomes. Key responsibilities include assessing inpatients for post-discharge services, collaborating with interdisciplinary teams, following up on plans from social workers, making referrals, and documenting discharge planning activities. The position requires a Bachelor's degree in social work, 1-2 years of experience in a hospital setting, along with strong interpersonal and clinical skills.
Required Qualifications
- Bachelors degree in social work from a college accredited by the Council on Social Work Education
- 1-2 years experience in a hospital setting
- Excellent interpersonal skills and effective communication
- Good overall clinical skills
- Ability to function well as a member of a multidisciplinary team
Desired Qualifications
- Preferably certified and/or working towards a Master’s degree
- Proficiency with Microsoft products such as Word, Excel, PowerPoint
