Job Summary
Social Worker - Case Manager provides psychosocial screening and assessment, targeted interventions, and care coordination to support children and families. Collaborates with healthcare and social services partners, conducts home visits as appropriate, facilitates access to community resources, maintains documentation, and participates in interdisciplinary teams to implement and evaluate a social work plan of care. Day shift on-site at Prisma Health in Greenville, SC, with responsibilities including identifying social determinants of health needs (housing, employment, mental health, substance abuse, wellness), coordinating interventions, outreach and community engagement, and contributing to quality improvement initiatives.
Required Qualifications
- Master's degree in Social Work
- Licensed Master of Social Work (LMSW) or Licensed Independent Social Work (LISW)
- Two (2) years of experience in community social work and working with children/families
