Social Worker - Case Manager Full Time / Day Shift

Assess patients for discharge planning needs within 24 hours of admission and coordinate care from admission to discharge. Attend multidisciplinary huddles to identify barriers, educate patients and families on discharge plans, and obtain timely authorizations for post-acute services. Conduct psychosocial assessments for high-risk populations, intervene with abuse cases, and mobilize community resources to prevent readmissions and improve outcomes. Delegate tasks to unlicensed team members, mentor colleagues, and ensure all documentation and regulatory compliance meet Case Management protocols. Full-time day shift at Melrose/Wakefield Hospital with a base pay range of $37.18 - $47.41.
