Job Summary
Conduct psychosocial assessments, provide crisis intervention, and advocate for residents' rights and dignity. Develop individualized service plans, coordinate care with physicians and community agencies, and facilitate family meetings regarding health status changes and end-of-life planning. Maintain accurate confidential records, ensure compliance with New York State Department of Health regulations, and participate in interdisciplinary care conferences. Requires a Bachelor's degree in Social Work, two years of experience, and a New York State license. Full-time position with competitive pay of $25.00–$26.00 per hour and comprehensive benefits.
Required Qualifications
- Bachelor's degree in Social Work (BSW), Human Services, Psychology, Sociology, or a related field
- Minimum of two (2) years of experience in social work, case management, assisted living, long-term care, healthcare, or senior services
Desired Qualifications
- Master's degree in Social Work (MSW)
- New York State Social Work license (LMSW or LCSW)
- Strong knowledge of community resources, discharge planning, and resident advocacy
- Excellent communication, organizational, and interpersonal skills
- Ability to manage multiple priorities while maintaining professionalism and confidentiality
- Proficiency in Microsoft Office and electronic documentation systems
