Job Summary
Assess patients' biopsychosocial needs through review of information, personal contact, and interdisciplinary team members. Communicate care plan status, progress toward goals, and problem-solving strategies to patients, families, and the care team. Intervene regarding emotional, social, and financial consequences of illness or disability, while providing counseling on treatment and end-of-life decisions. Evaluate psychosocial risk factors based on prior functional levels and support system adequacy. Provide discharge planning and continuity of care for assigned patients in acute and post-acute settings. Initiate referrals to clinics, home healthcare, hospice, SNF, acute rehab, LTAC, TCM, and medical equipment. Collaborate on discharge planning needs and transition patients to community care settings. Document relevant discharge information in the medical record and act as a liaison between patients, families, and interdisciplinary members.
Required Qualifications
- Basic Life Support certification upon start
- 1 year experience in clinical care or clinical case management
- Master's Degree Social Work (MSW)
- Full-time employment
- Day shift with varying hours from 7:30 a.m. – 5:30 p.m.
- Monday – Friday schedule
- Location in the Fairfax area
- Working knowledge in utilization management, managed care and payer issues
- Understanding of pre/post-acute resources
- Ability to assess psychosocial risk factors of patients/families
- Ability to intervene with patients/families regarding emotional, social and financial consequences of illness and/or disability
- Ability to provide counseling and interventions related to treatment and end of life decisions
- Ability to advocate for patient/family empowerment and independence to make autonomous healthcare decisions and access needed healthcare services
- Ability to initiate and facilitate referrals to clinics, home healthcare, hospice, SNF, acute rehab, LTAC, TCM, medical equipment and supplies
- Ability to collaborate with the interdisciplinary care team, patients and families in the assessment/coordination of discharge planning needs
- Ability to document relevant discharge planning information in the medical record according to department standards and/or care management plans
- Ability to act as a key Liaison between patients, families and the interdisciplinary healthcare members
Desired Qualifications
- Previous Pediatric Specialists of Virginia (PSV) work experience
