Social Worker - case Management

Job Summary
Provide ongoing case management support to patients. Assess patients' needs and develop discharge care plans. Communicate effectively with staff, colleagues, patients, and family members. Utilize standard Social Work intervention techniques in collaboration with patients and families. Work as a resource to other staff members. PER DIEM Social Worker in a hospital Case Management team responsible for assessing and providing direct Social Work services and interventions to patients and families upon admission and throughout discharge.
Required Qualifications
- MSW degree from an accredited School of Social Work
- LMSW or LCSW preferred
- Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred
- Knowledge of Medicare, Medicaid, and managed care regulations and trends
- Experience with utilization management, home care, community resources, and rehabilitation beneficial
