Social Worker - Case Management - FT - 1st Shift

Facilitate resource management activities for inpatients and coordinate patient transitions from hospital to home through collaboration with physicians, nursing staff, and healthcare professionals. Provide psychosocial assessments, resources, and interventions to hospitalized patients to enhance continuity of care and promote cost-effective utilization of resources. This full-time, first-shift role requires a BS degree in Social Work with current Alabama licensure and two years of hospital experience. The team supports diverse populations while demonstrating emotional resilience during crises.
