Social Worker Case Management

Assess complex emotional and social barriers to develop personalized care plans addressing each patient's unique needs. Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care. Advocate for patient rights and preferences within the medical system to ensure person-centered care. Empower patients and families through education on financial and community resources to help them build stability for managing their diagnosis. Requires a Master's degree in Social Work, Social Worker license, and Certified Case Manager or ACM certification. 3 years of experience preferred. Schedule includes Sundays and Mondays, 2x10 hours day shift.
