Social Worker

Participate in an interdisciplinary team approach to better serve the Center population by providing therapeutic intervention for residents' social and psychological needs. Perform initial evaluations of psychosocial needs, develop social service components of care plans, and coordinate care planning conferences at admission and required intervals. Maintain written documentation in resident medical records, ensure family input into care planning, and act as a liaison between residents, families, outside agencies, and the Center Administrator. Facilitate discharge planning, conduct family meetings, coordinate admissions through referrals, and verify insurance coverage. Conduct facility tours for outside interested parties while ensuring resident safety. Requires a Master's Degree in Social Work, CSW or LSW license, and one year of experience.
