Job Summary
Social Worker to support young people and adults transitioning from children’s services into adult care; conduct comprehensive assessments and create tailored transition plans; provide guidance and advocacy to young people and their families; collaborate with education, health, and social care professionals; ensure safeguarding policies and care legislation are followed; maintain accurate records and provide detailed reports to inform decision-making; promote independence, inclusion, and person-centered care approaches.
Required Qualifications
- Hold a social work degree.
- Right to work in the UK and hold clean UK driving license.
- Registered with SWE
- Post qualified experience within children and family services.
