Job Summary
Social Worker/Care Manager in Pediatric Care Management coordinates care for pediatric patients and families, conducts assessments of clinical and social status, collaborates with providers, staff, and community resources, educates providers about patient needs, participates in in-person or Telehealth visits, maintains HIPAA-compliant confidentiality, and advocates for children across systems of care while joining an interdisciplinary team to support families and ensure patient-centered care.
Required Qualifications
- BS in Social Work or Social Services required for Pediatric Care Managers
- MHRT-C preferred (or eligibility for MHRT-C)
- Minimum of three years direct experience in social work in a health care setting
- Valid driver’s license with acceptable driving record
- Up-to-date training and certification in Healthcare Provider Basic Life Support (BLS) or ability to obtain within 2 weeks of hire
