Job Summary
Identify and prioritize patients requiring social services using a holistic biopsychosocial approach. Plan discharge interventions with patients, caregivers, and the healthcare team to ensure smooth transitions across the continuum of care. Monitor patient conditions and revise plans as needed while maintaining clear, timely documentation that reflects post-hospital resource needs and caregiver involvement. Supervise peers, social workers, and students, and support denial prevention by removing barriers to timely testing and treatment. Communicate treatment goals and preferences to the multidisciplinary team to address readmission prevention and length of stay management.
Required Qualifications
- Bachelor of Social Work
- BLS Basic Life Support – American Heart Association
- Licensed as a Social Worker in state of practice
- 1 year of experience in clinical setting
Desired Qualifications
- Master's degree in social work or healthcare related field
- Licensed as a Social Worker in state of practice (VA)
- Accredited Case Manager Certification (ACM) from American Case Management Association
- Certified Case Manager (CCM) from Commission for Case Manager Certification
- 3 year of experience in an acute care clinical setting
- Ambulatory or post-acute, care coordination experience
