Job Summary
Provide emotional support and brief therapy to residents in skilled nursing facilities; conduct mental health assessments; document interactions and interventions; collaborate with psychiatric professionals as part of a Collaborative Care team; support geriatric mental health and psychosocial needs through short-term, solution-focused interventions; participate in monthly clinical meetings with psychiatric providers; nursing/long-term care residents to be seen approximately 15 hours per week with a flexible schedule.
Required Qualifications
- Associate's degree in Social Work, Psychology, Counseling, or another mental health-related field
- Minimum 1 year of experience
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
