Job Summary
Social Worker/Care Companion providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities as part of Carease Health's Collaborative Care team in Cleveland, OH. Responsibilities include visiting designated facilities, conducting psychosocial assessments, implementing short-term, solution-focused therapy, documenting notes, and collaborating with psychiatric professionals (monthly meetings with a psychiatric NP). Requires an Associate Degree in a mental health-related field, understanding of geriatric mental health and crisis management, strong communication and documentation skills, and 1 year of related experience. Flat salary of $11,000 - $13,000 annually with approximately 10 hours per week, Monday-Friday, 8-5 hours; paid training and 401k available.
Required Qualifications
- Associate Degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- 1 year of experience in a related field
