Job Summary
Provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Visit designated facilities, conduct assessments to identify emotional, cognitive, and psychosocial needs, implement short-term, solution-focused therapy and supportive interventions, and document visit notes with detailed records. Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
Required Qualifications
- Associate, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- 1 year of experience in a similar role
