Job Summary
Social Worker/Care Companion provides emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Visit designated facilities as scheduled, conduct assessments to identify emotional, cognitive, and psychosocial needs, implement short-term, solution-focused therapy, document visit notes, and collaborate with psychiatric professionals including participation in a monthly meeting with the psychiatric NP. Flexible, part-time scheduling with paid training and supervision within a supportive clinical team.
Required Qualifications
- Associate Degree in Social Work, Psychology, Counseling, or another mental health–related field.
- Minimum 1 year of experience.
