Social Worker, BSW

Social Worker, BSW position onsite at SSM Health in Clayton, MO, night shift (7:00 PM–7:00 AM) with rotating every third weekend. Provides social work assistance to patients and families related to social determinants of health; interviews patients, assesses priorities, documents case activity, and collaborates with the multidisciplinary team to coordinate community resources and treatment. Develops and maintains relationships with community health, welfare, and social agencies to assist patients with needs; refers to resources for community assistance; participates in opportunities for standard-work improvement. Requires organization, responsiveness, and comfort working in a fast-paced overnight environment. Education: Bachelor's degree in Social Work. Experience: No experience required. Location is onsite in Missouri (Clayton) with 36 hours/week. Equal opportunity employer.
