SOCIAL WORKER - BROWARD COUNTY - PER DIEM

Job Summary
Instruct, treat, observe, and evaluate patients with significant social and emotional problems affecting their health status in their homes. Perform psychosocial assessments, counsel families on coping with illness, and implement the physician's Plan of Care including supportive counseling and community referrals. Assess pre-bereavement needs and apply infection control plans to protect patients and staff. Complete timely documentation, participate in in-service programs, and maintain required licenses and continuing education. Work Monday to Friday 8am to 5pm with alternating weekends, lifting up to 50 pounds. Requires a Master's degree in Social Work, active Florida license, and 1-2 years of healthcare experience.
Required Qualifications
- Master's Degree from a school of Social Work accredited by the Council on Social Work Education
- Clear and Active Professional License to practice in the state of Florida
- 1-2 years of social work experience in a health care setting
- Knowledge of AHCA Regulations, Medicare COP's Guidelines
- Knowledge of computer office/clinical software
- Must be able to read, write and understand the English language
- Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds
- Must be able to walk
- Must be able to climb stair or ladders
- Must be able to stand on feet for extended periods of time
- Must be available for alternating weekends
Desired Qualifications
- Home care experience
- Knowledge of public health administration and/or social work consultation and/or supervision
