Job Summary
Provide behavioral health services within primary care sites as part of the collaborative care model. Conduct psychosocial assessments and implement evidence-based behavioral health interventions and psychoeducation. Track and analyze data in patient registries to support care coordination across settings with the patient, family, and care team. Facilitate in-person or virtual huddles, develop care plans, and connect patients with community resources. Document assessments and interventions in the electronic medical record, support groups, and patient education adapted to patient and family learning styles using teach-back. May supervise graduate social work interns (SIFI eligible). This is a Full-time position with Monday–Friday, 9:00 am–5:00 pm hours in Brooklyn, NY. Preferred: at least three years of social work experience, NYS LCSW license, SIFI certification, experience with mental illness, bilingual English/Spanish preferred.
Required Qualifications
- Master’s Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW)
- If a new graduate Social Worker, Limited Permit (must supply exam date prior to start)
- If a new graduate Social Worker, must have Licensed Masters Social Worker (LMSW) by the State of New York within 1 year of hire
