Job Summary

Social Worker/Bereavement Coordinator provides psycho-social and bereavement support to hospice patients and families, participates as a member of the interdisciplinary team to develop and implement care plans, promotes advance care planning (including living wills and POA), coordinates bereavement services, and supports community resources. Requires a Bachelor's degree in Social Work (or related field) with one year of healthcare social work experience and Alabama licensure requirements; preferred Master’s degree and additional grief/bereavement experience. Must be able to work a flexible schedule including nights and weekends; supervision by MSW for hospice planning and counseling needs. The role emphasizes nondiscrimination, ethical conduct, and engagement with external stakeholders, along with documentation, policy adherence, and ongoing professional development.

Required Qualifications

  • Bachelor's degree in Social Work, Psychology, Sociology, or related field with one (1) year of social work experience in a healthcare setting; licensure if required by state rules; supervision by MSW for hospice patient care planning and counseling needs; State-specific licensure in Alabama to title/practice social work (LBSW, LMSW, LCSW, or LICSW)
