Social Worker

Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations. Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers. Build and maintain positive, supportive relationships with residents and their families. Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments. Develop, implement, and update individualized psychosocial care plans for residents. Participate in care planning meetings and support continuity of care throughout the resident's stay. Provide counseling, emotional support, and resource referrals as needed. Ensure compliance with all applicable facility policies, regulations, and documentation standards. Full-time position with annual salary $75K-85K. Bachelor's degree required; Master's degree strongly preferred. Experience as a Social Worker in a healthcare-related setting required. Located at Atlantic View Post Acute and Healthcare Center, a 154-bed Sub-Acute and Long-Term Care facility.
