Job Summary
Assess and support citizens on discharge from hospital across three pathways, completing post-discharge assessments and reviews in line with the Care Act. Undertake Care Act Assessments (CAA) and support planning, while carrying out Best Interest (BI) decisions within the Mental Capacity Act (MCA). Manage safeguarding concerns in line with statutory responsibilities and provide duty cover where required. Attend Quality Assurance (QA) panels to support funding decisions. Required Qualifications
- Qualified Social Worker with relevant Social Work England registration
- Experience within Adults Services, ideally discharge or hospital-based teams
- Strong knowledge of Care Act, MCA, and safeguarding legislation
- Ability to work in a fast-paced environment and manage competing priorities
