Job Summary
Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs. Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans addressing psychological concerns. Provide counseling, emotional support, and crisis intervention for grief, family conflicts, or health changes. Serve as an advocate for resident rights and connect families with community resources for financial and legal needs. Work weekends in West Hartford, CT, with a Master's degree in social work and current licensure required.
Required Qualifications
- Master's degree in social work from an accredited program
- Current licensure or certification as a Social Worker in the state of employment
- Previous experience in Long-Term Care, geriatrics, or a related field
- Strong assessment, counseling, and crisis intervention skills
- Excellent communication and interpersonal abilities
- Ability to work collaboratively as part of a multidisciplinary team
- Empathy, compassion, and a commitment to person-centered care
