Social Worker

As an Inpatient Medical Social Worker at Aurora St. Luke’s Medical Center, provide professional social work services and discharge planning through psychosocial assessments and coordination of post-hospital care needs. Collaborate with interdisciplinary teams to coordinate safe, timely care across the continuum, including post-acute services and community resources, while advocating for patient-centered care. Educate patients and families on healthcare decisions, available resources, and complex issues such as advanced directives and end-of-life care. Connect individuals to community services, ensure continuity of care and smooth transitions, support care coordination to reduce length of stay and readmissions, and maintain thorough documentation while complying with regulatory standards. Travel to support site requirements; schedule includes 40 hours per week with possible weekends/holidays.
