Job Summary
Social Worker in hospice care delivering psychosocial assessments, developing and updating the Plan of Care with the Interdisciplinary Team, assisting patients and families with insurance needs (including Medicaid), coordinating with volunteers, participating in discharge planning, bereavement work, and after-death follow-up, and serving in an on-call rotation for after-hours coverage. Responsibilities include conducting psychosocial assessments within five days of admission, documenting interventions and outcomes in the EMR, coordinating with IDT to formulate the POC, communicating changes to the attending physician, and supporting community outreach and resources for patients and families.
Required Qualifications
- MSW preferred; BSW required in certain states (SC)
- One year of social work experience in a health care setting
- Social Work Licensure as LMSW/LBSW depending on state
- Ability to discuss Plan of Care with admitting RN on admission
- On-call rotation availability for after-hours coverage
- Knowledge of Hospice Benefit and ability to convey to patients/families
