Job Summary
Provide administrative and clinical supervision to the Care Management team, including Care Coordinators and Case Assistants. Monitor service provision, referrals, census, and ensure timely monthly billing. Conduct staff training, performance appraisals, and assist with hiring and termination. Facilitate weekly team meetings, case conferences, and crisis intervention services while coordinating 24-hour coverage. Review assessments, plans, and documentation to ensure compliance and quality assurance. Assist with admission and discharge decisions and lead community outreach efforts. Travel within New York City for 30% of the time to engage members in homes and community settings. Requires a Master's degree in social work or related field with four years of case management experience.
Required Qualifications
- Masters Degree in Social Work, Nursing, Public Health, Counseling Psychology, Psychology, Psychiatric Rehabilitation, Education, Public Administration or other related social service or health profession
- At least four years of experience in providing case/care management services to persons with disabilities (medical, psychiatric or cognitive disabilities) or comparable experience
- Experience working in interdisciplinary teams
- Experience providing case management or care coordination in a medical or behavioral health environment
- Experience working with the chronically ill
- Licensure in a related field of practice (LCSW, LMSW, LMHC)
- Intermediate computer proficiency
- The ability to read paper and electronic documents
- The ability to perform significant data entry into various computer programs
- Manual dexterity and hand-eye coordination
- The ability to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds
- The ability to sit (70% of the time)
- The ability to stand (20% of the time)
- The ability to travel to and from appointments using varied public and private transportation options (10% of the time)
Desired Qualifications
- Excellent verbal and written communication skills
- Strong customer service skills
- Fluency in a second language such as Spanish, Russian, or Creole
- One year of supervisory experience
