Job Summary
Social Work Supervisor directs the Care Management team to ensure day-to-day operation of the program under the Director of Care Management, supervising Care Coordinators and Case Assistants (Peer Specialists) and, when needed, performing duties of Care Coordinators for assigned enrollees. Responsible for administrative and clinical supervision, program compliance, billing oversight, staff training, admissions/discharges, and quality assurance activities. Leads crisis intervention, coordinates care for high-risk cases, facilitates weekly team meetings, and supports program expansion initiatives. Requires a Masters degree in a related field, licensure (LCSW/LMSW/LMHC), four+ years in case/care management, and preferred bilingual capabilities in Spanish, Russian, or Creole; one year supervisory experience is preferred. Based in Staten Island, NY with in-person work requirements; travel within New York City may be required. The role emphasizes interdisciplinary collaboration, community outreach, and maintaining high-quality service delivery across clinical and administrative domains.
Required Qualifications
- Masters Degree in Social Work, Nursing, Public Health, Counseling Psychology, Psychology, Psychiatric Rehabilitation, Education, Public Administration or other related social service or health profession
- At least four years of experience in providing case/care management services to persons with disabilities (medical, psychiatric or cognitive disabilities) or comparable experience
- Experience working in interdisciplinary teams; experience providing case management or care coordination in a medical or behavioral health environment; experience working with the chronically ill
- Licensure in a related field of practice (LCSW, LMSW, LMHC)
- Fluency in a second language such as Spanish, Russian, or Creole preferred
- One year of supervisory experience preferred
