Job Summary
Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department environments. Promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources through collaboration with service team members, physicians, patients, external agencies, and families. Requires a Master's Degree in Social Work from an accredited program and one year of experience in an acute or post-acute setting. LMSW, ACM, and CCM certifications are preferred.
