Social Work Clinical Leader – Cerebral Palsy Program

Job Summary

Lead the Clinical Medical Social Worker within the interdisciplinary team to improve social, emotional, functional, and physical health of pediatric and adult patients in the Cerebral Palsy program. Conduct assessment and intervention for barriers to care, acute safety concerns, and transition-to-adulthood planning while providing direct supervision to other social workers. Collaborate with leadership on QI initiatives, program development, and departmental SW initiatives to ensure ethical decision-making and appropriate documentation. Manage patient and family coping with stress as they navigate healthcare and community systems.

Required Qualifications

  • MSW
  • LISW
  • current Ohio Social Work license
  • NASW Code of Ethics for Social Work
  • 8am-4:30pm, M-F
  • direct supervision to other social workers
  • contributes to SW department leadership initiatives
  • active member of the CP Leadership Team
  • supporting QI initiatives and program development
  • Models and leads the Clinical Medical Social Worker within the interdisciplinary team
  • improve or maintain social, emotional, functional, and physical health of patients and families
  • Ensures ethical and sound decision-making
  • appropriate documentation
  • appropriate interpersonal communication of the social workers for whom they are responsible
  • Collaborates with Clinical Medical Social Work leadership, internal stakeholders and other affiliated teams
  • development, implementation, and evaluation of Clinical Medical Social Work services
  • assessment & intervention for barriers to care
