Social Work Case Manager

Inpatient Social Work Case Manager collaborates with the healthcare team to coordinate post-hospitalization services, conducts psychosocial assessments across patient/family contexts, facilitates discharge planning (including home care, nursing home placement, and access to community resources), communicates treatment plans to stakeholders, adheres to regulatory requirements, actively participates in interdisciplinary care coordination, and provides supportive counseling to patients and families to support adjustment to disability and illness while ensuring patient-centered care and timely documentation in the medical record.
