Job Summary
Social Work Case Manager responsibilities include navigating social determinants of health to connect patients with benefits, housing, transportation, and social services; collaborating with an interdisciplinary care team to develop individualized case management plans; conducting biopsychosocial assessments; advocating for patients, facilitating referrals to community resources and public programs, and maintaining thorough documentation and quality reporting; requires trauma-informed and harm-reduction approaches, effective de-escalation skills, and strong interpersonal abilities; bilingual Spanish or Hmong preferred.
Required Qualifications
- High school diploma or GED
- Bachelor’s degree in social service or health-related field or an equivalent combination of education and experience
- Two years of experience in a similar role
- Bi-lingual competency in Spanish or Hmong preferred
