Job Summary
Social Work Case Manager in MarinHealth leads multidisciplinary care plan development and discharge planning for patients, acting as a key liaison among physicians, staff, and hospital leadership to ensure appropriate level of care, quality outcomes, and efficient utilization. Responsibilities include psychosocial assessments, individual/conjoint/family therapy as appropriate, advocacy and resource coordination, facilitation of multidisciplinary case conferences, and documentation in alignment with CMS/state regulations. Requires an LCSW, demonstrated leadership and communication skills, knowledge of NASW Code of Ethics, HIPAA, and healthcare regulations, and the ability to coordinate complex psychosocial and discharge planning needs across age groups.
Required Qualifications
- Master's degree from an accredited school of social work or social welfare required
- Licensed Clinical Social Worker (LCSW) required at hire
- BLS within 90 days of hire
- IAMTAC within 30 days of hire
- ACM-SW preferred at hire
- C-SWCM preferred at hire
- Knowledge of CMS, DHCS, Joint Commission, EMTALA, Title 22, DOJ regulatory guidelines
- Knowledge of Medi-Cal, Medicare, DRGs, managed care, capitation, and reimbursement requirements
- Experience meeting the needs of diverse patient populations across age ranges
- Strong clinical assessment and discharge planning skills
