Job Summary
Provide comprehensive case management for home and community-based services to older adults, children, and individuals with disabilities. Collaborate with a Care Management Team (CMT) including MSW/BSW and RN to coordinate services, resources, and supports; assist with Medi-Cal, IHSS, CalFresh, and Regional Center applications; advocate to ensure timely medical services; educate participants about available services; document interactions and maintain timely reporting; participate in daily case conferences with CMT; required to work 8am–5pm on weekdays from the Chico office with travel to homes in Butte County and surrounding areas. Requires strong problem solving, knowledge of community resources, and ability to coordinate transportation and basic supports; CPR within 30 days; valid CA driver’s license and own vehicle; PPE provided; travel is local to client homes.
Required Qualifications
- Master's degree or Bachelor’s degree in Social Work or another related field
- Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
- CPR certificate required within 30 days of hire
- Experience with electronic medical records
