Job Summary
Social Work Case Manager at UChicago Medicine AdventHealth in Bolingbrook, IL. Required Qualifications
- Master's degree in Social Work (MSW)
- Active state LSW
- Active state LCSW (Preferred)
- Accredited Case Manager (ACM) (Preferred)
- Certified Case Manager (CCM) (Preferred)
- 2+ care management experience (Preferred)
- 2+ hospital social work experience (Required)
- Computer proficiency with Outlook and EHRs (Required)
- Discharge planning experience (Required)
- Strong interpersonal and negotiation skills (Required)
- Knowledge of state and federal guidelines pertinent to care management (Preferred)
- Faculty of care coordination and post-acute care knowledge (Required/Preferred)
- Licenses and Certifications: LSW/LCSW (Required/Preferred)
- Education: Master's (Required)
- Work Experience (2+ years) in care management (Preferred)
- Discharge planning and care transitions expertise (Required)
- Patient advocacy and family education (Required)
