Social Work Care Manager I, Case Management

Facilitate discharge planning and address complex care needs for patients at risk of adverse outcomes due to limited resources, family dynamics, mental health issues, or substance use disorders. Perform mental health assessments, evidence-based interventions (SBIRT), cognitive function screenings, crisis interventions, safety planning, and other appropriate psychosocial interventions. Collaborate with Nurse Care Managers, physicians, nursing staff, and other team members to determine when social work involvement is indicated to address patient care needs. Requires a Master of Social Work, previous experience in medical social work, and BLS certification within 30 days of hire.
