Social Work Care Manager

Job Summary
Coordinate patient care from admission through discharge by completing thorough psychosocial assessments, attending interdisciplinary rounds, and facilitating high-quality clinical and cost outcomes. Act as the primary advocate for patients and families, balancing their preferences with care coordination needs while identifying barriers to efficient discharge planning. Secure post-acute services through timely referrals, manage payor communications to reduce denials, and ensure compliance with federal and state regulatory requirements. Escalate complex cases to management, Physician Advisors, or Ethics committees as appropriate. Provide therapeutic communication to support patients experiencing anxiety or stress due to illness or injury.
Required Qualifications
- Valid, unencumbered Registered Nurse License approved by the Georgia Board of Nursing
- Two (2) years experience in healthcare
- Masters in Social Work from an accredited school of social work
- Licensed as a Master Social Worker (LMSW) through the Georgia Composite Board of Professional Counselors, Social Workers, and Marriage & Family Therapists
- Demonstrated knowledge of software/EMR applications
Desired Qualifications
- Bachelors degree in Nursing from an accredited school of nursing
- One (1) year healthcare experience in Acute Care setting
- Accredited Case Manager (ACM) Certification through the American Case Management Association or Certified Case Management (CCM) through the Commission for Case Manager Certification
- Two (2) years healthcare experience in Acute Care setting
- Three (3) years healthcare experience in Acute Care setting
