Job Summary
Social Work Care Manager I in the Emergency Department performs psychosocial assessments, develops and monitors care plans, and provides crisis intervention, individual/family therapy, and referrals. The role collaborates with patients, families, healthcare teams, insurers, and community resources to facilitate safe transitions of care, address social determinants of health, and advocate for access to services. Responsibilities include assessment and screening for transition planning, behavioral health support using motivational interviewing and trauma-informed care, brief therapy (individual, group, and family), education and staff training, and leading quality-improvement initiatives. Requires an MSW with state licensure and BLS for caregivers who visit homes/communities; must have a valid driver’s license, auto insurance, and reliable transportation. Shift: every other weekend, 6:00 PM–6:30 AM. Location: St. Marys Regional Hospital, Grand Junction, CO. ,
Required Qualifications
- Master of Social Work (MSW) from an accredited institution (degree verification required)
- Current state licensure, as applicable, obtained prior to or upon completion of required supervision hours
- Basic computer proficiency, including familiarity with word processing and spreadsheet software
- Strong written and verbal communication skills
- Demonstrated ability to apply critical thinking skills
- Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation
