Job Summary
Seeking a bilingual Social Work Care Manager to provide telephonic and in-home assessments for members' medical, psychosocial, physical, and spiritual needs. Ideal candidates will have a Bachelor's degree in social work or a related field, with a Master's degree preferred, and must possess strong interpersonal and assessment skills.
Required Qualifications
- BS in social work or related field
- Current NY State CSW for Master's prepared candidates
- Prior human services experience
- Demonstrated assessment skills
- Strong communication and interpersonal skills
- Strong comprehensive care planning and crisis intervention skills
- Access to a vehicle for local travel
