Job Summary
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs in designated skilled nursing facilities. Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns while documenting visit notes and maintaining detailed records of interactions. Visit facilities in accordance with the schedule set with your supervisor and collaborate with psychiatric professionals, including participation in monthly meetings with the psychiatric NP. This role offers flat salary flexibility, paid training, and professional growth through ongoing clinical meetings and supervision.
Required Qualifications
- Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- Associate Degree
