Job Summary
Provides psychosocial support and direct care as part of the interdisciplinary team (IDT); manages a caseload and coordinates services and care plans in collaboration with participants, families, and IDT members; conducts home, hospital, and skilled nursing visits; completes necessary paperwork, grievances, appeals, and incident reports, and helps with quality and compliance requirements.
Required Qualifications
- High school diploma or equivalency
- Associate degree preferred
- CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
- Reliable means of transportation
