Job Summary
The Social Work Assistant (Social Worker I) is a field and office-based role responsible for screening and assessing Home Delivered Meal clients. The position involves working with potential program applicants to provide information, conduct screening, and complete intake and enrollment paperwork for homebound older adults. The assistant also supports the Home Delivered Meal Supervisor with administrative duties, conducts quarterly home and phone assessments, and contributes to other program components as assigned. Strong communication, time management, attention to detail, and computer/organizational skills are essential, as is the ability to interact with individuals at all levels and handle a large volume of client assessments.
Required Qualifications
- Bachelor’s Degree from an accredited university in human services or two (2) years of experience in homecare or a related field
- Current unrestricted CA Driver’s License and proof of auto insurance
- Experience and interest in providing health services to older adults
- Experience with Electronic Health Records systems desired
- Must be computer literate in MS Office applications (Word, Excel, and Outlook)
- Demonstrated ability to work in an interdisciplinary community setting maintaining professional boundaries
- Excellent communication skills both verbal and written and ability to utilize telehealth platforms
- Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment
