Job Summary
Provide social work support to ambulatory care patients at Harborview Medical Center, assisting patients in accessing medical and financial benefits (DSHS, Washington Health Plan Finder, Expanded Medicaid, SSA, Medicare), conducting comprehensive financial assessments, advocating with Social Security/DSHS, and guiding patients through payer options. Maintain HIPAA-compliant documentation, develop individualized treatment plans, and support clinic Social Workers with referrals to in-home services, transportation, housing navigation, and other community programs. Promote ethical practice per NASW Code of Ethics and serve as an ambassador for the Social Work Department, strengthening relationships within the service area.
Required Qualifications
- Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent
- OR an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent
