Social Work and Discharge Planning Assistant

Job Summary
Document care plans and services in resident medical records per state and federal requirements. Complete Minimal Data Set sections and arrange durable medical equipment, clothing, and personal items for residents. Coordinate home health, hospice, and supportive services by making referrals to outside agencies and assessing support systems for discharge planning. Act as resident advocates by educating families on rights and ensuring comprehensive psychosocial care plans are updated quarterly. Initiate discharge planning upon admission, schedule interdisciplinary meetings, and facilitate bed management to maximize transition services. Communicate regularly with residents and families regarding needs and psychosocial history.
Required Qualifications
- Bachelor's degree in social work, human services, or related field
- Two (2) years of experience in Social Work or related role
